Event Documents
If you are attending one of the DATR events, please fill out the following forms. Read below for which documents need to be brought with you to the event.

All attendees need to print, fill out, and bring the following three forms with you the day of the event:

If you are bringing a minor with you who is not your child, the following document needs to be printed out, filled out by the parent, and brought with you the day of the event:
*In the case you do not have a printer, we will have extra forms you can fill out upon arrival. In which case, please plan to show up early to give yourself time to fill it out. It is easier and faster if it is already done when you arrive.
